It was low transmission fluid for me. As JustCody was saying, the inertia builds pressure in the drivetrain so you need high pressure to downshift but if fluid is low you will have low pressure.
I've heard that it takes about 12 seconds for your oil pump to fully lubricate your engine... so I'd think there isn't much benefit to let it run much longer than that.
I wait maybe 15-30 seconds. Once the RPM drop and the rad fan disengages I put it in drive. If it is really cold like -20°C I will let it idle for a minute or two while I clean the truck off. Any longer than that and you're just wasting gas.

Hmmm well the fact that you didn't have the issue until you removed the exhaust, and O2 sensor would lead me to believe that it has something to do with the O2 sensor.

How about your fuel filter? I've heard these trucks are notorious for the fuel filter degrading and then clogging the system. Lets hope its not that.
