Intermittent rattling when using the throttle while in Drive.

Couldn't figure out where this noise is coming from. It had a recall work done on the U-Joint shaft about 2 years ago. can't remember when this rattling started. it's definitely not the exhaust. it doesn't rattle when in park/neutral. It randomly does it when i lightly press on the gas at low RPM. when i really step on it, i don't really notice any rattling.

While not in a Toyota, I diagnosed an early 80's Chevy with that type of noise. Drove me crazy, found it to be a baffle in the muffler that had broken loose. The muffler itself had a small leak from a split seam, so I replaced the muffler. When I took the muffler off, it fell on the ground and made the rattle noise. New muffler, no noise.

The noise was surprisingly loud for being inside a muffler.

No,but it seems like something the dealership or Toyota would say. But I do have the same rattle when engine is cold and you slowly throttle

Oh without a doubt that is in the "Toyota dealer book of excuses". # 4 or #5 I believe.

If you are old enough, you will remember the heat riser valve on US engines rattling after they got some mileage on them. The valve obstructed the exhaust flow in order to warm the bottom of the intake manifold and prevent lean stumble in cold weather. As far as I recall, the heat riser valve was removed when the cars went to fuel injection/throttle body injection. Yes, I'm old. LOL...

Did you find the noise? I too have a similar problem when I lightly throttle at low rpm where the engine would vibrate just right to hear a rattle that sounds like a bearing going bad from the engine bay on my 2013 prerunner
