I'm hearing noise like a rattling when I shift from park to D or R. Is this a u joint? when I start moving at 5mph the noise subsides :unsure:

My PreRunner had that noise, check your exhaust, I had part of the inside clamp break off and was just rattling on the the rest of the exhaust. I ended up cutting it off, it was like a big o-ring on the tubing that vibrated when the engine was running. Really annoying. My wife's escort did the same thing.

Can you tell if when you go above 5mph the noise stops or if road noise just covers the noise

I no longer have that truck, Toyota destroyed it. However, once I got out on the road I couldn't hear it anymore. It was mostly when I 1st started it, Sometimes it would slide back into position and I wouldn't hear it at all. Check all your clamps and fittings on your exhaust to make sure none are loose or rusted and parts of it broken off.

I had the same kind of thing happen to my truck it was just a bolt came out of a heat shield under the truck and it was rattling, I couldn't hear it once I got moving also
